For years, common sense held that firmer mattresses helped people with back problems, a view bolstered by anecdotal information. But studies suggest otherwise. Firm mattresses aren’t always better for your back. In fact, the opposite is probably closer to the truth.

There wasn’t ever any solid evidence to support sleeping on the floor or a very firm mattress. Despite this, orthopedic specialists used to recommend it to patients. And this study, conducted in Spain, shows that there are factors involved.

The study looked at 313 adults who all experienced undiagnosed chronic lower-back pain. All of them had complained about pain in their lower backs when lying in bed. They were given new spring mattresses without knowledge of the type of mattress. About half were given firm mattresses and half were given medium-firm mattresses. After three months, researchers discovered that those who slept on medium-firm mattresses were twice as likely to report improvement in low-back pain than those who slept on firm mattresses.

Why was the less firm mattress better for backs? A few reasons. First, some of the people just needed a new mattress, which are better for backs. The study also show that medium firm mattresses better distribute pressure. Firmer mattress don’t contour to the body. On the other hand, soft mattresses can contour too much.

Participants in the study reported an improvement after merely sleeping on a new mattress. This tells us that not one mattress is right for everyone and it’s important to replace a mattress when you need to.
